About Niche

Niche Medical is a limited-scope adult medicine primary care micropractice in Carmel, Indiana with a focus on prevention of illness and disease and partnering with patients to help them achieve optimal metabolic health and promote longevity. Niche Medical will best serve patients who are generally healthy and hope to stay that way! Our lovely 200 square foot space is conveniently located in West Carmel, Indiana.

Flat Fee Annual Exam with Basic Annual Labs

  • Complete medical history, vital signs, and physical examination (*currently unable to support gynecological exams due to space and office limitations)

  • Includes basic annual labs and in-visit (when able) blood draw:
    • Complete blood count with differential (screen for anemia and blood disorders)
    • Comprehensive metabolic panel (electrolytes, kidney, and liver function)
    • Lipid panel including Apolipoprotein B (cholesterol screening)
    • Hemoglobin A1c and Fasting insulin (diabetes and metabolic health screening)
    • Thyroid stimulating hormone (thyroid function)
    • Option to add additional testing based upon medical need or patient request- all with prior consent / cost transparency

Pre-Operative Medical Assessment and Evaluation
60 minute appointment / $300
  • Complete medical history, vital signs, and physical examination necessary for pre-operative / pre-procedural safety
  • Includes onsite blood draws (when appropriate) and direct care lab pricing for those not using insurance
  • Includes prescriptions (if needed), referrals, orders for diagnostic testing
  • Labs and ECG (electrocardiogram for heart electricity evaluation) available onsite for an additional charge if indicated based upon evaluation or surgeon request
  • Includes written summary of recommendations and resources for pre-procedure / pre-surgical medical and medication instructions
  • Includes written evaluation sent directly to the requesting surgeon or physician performing the upcoming procedure

Comprehensive Longevity / Lifestyle Medicine Evaluation

Dr. Laurel Fick

After 19 years in practice as a board-certified Internal Medicine physician and medical educator in traditional US healthcare systems, Dr. Fick ventured into the direct care model to provide high quality, affordable, and patient-centered primary and limited-scope care to her patients. Her approach to medicine is centered around preventive and lifestyle medicine philosophies and a sturdy belief that the body is capable of self-care and healing with proper nutrition, exercise, sleep and stress management, social connection, and avoidance of risky substances. As such, she is pursuing board certification from the American College of Lifestyle Medicine and eventually hopes to have a lifestyle-medicine predominant practice.

Dr. Laurel Fick is an accomplished medical educator having served in medical residency leadership for 16 years. Her academic areas of interest are in physician wellbeing, particularly physicians-in-training and mentorship / advocacy. Dr. Fick serves as an elected Councilor for the Association of Program Directors in Internal Medicine, was elected to serve as a member of the Transitional Year Review Committee of the Accreditation Council for Graduate Medical Education (ACGME), and on the advisory committee for the Women Physicians Wellness Conference. Dr. Fick has presented and published widely on topics related to physician wellbeing and educational innovation.

Dr. Fick and her husband of 15+ years have two school-aged children, a houseful of pets, and a growing garden. In her spare time Dr. Fick likes to read, travel, coach her kids' basketball and volleyball teams, and explore her culinary potential.
